TREATING TYPE 1 DIABETES.....


Hi today we are going to discuss about how to manage type 1 diabetes.



Since type one diabetes is caused by autoimmune destruction of the pancreas, that results in an absolute deficiency of insulin, it makes sense that the treatment of type one diabetes is to give insulin. Now, this is true, but unfortunately it's not quite that simple. So let's talk about treating type one diabetes. And before we get into the specifics of the treatment, let's first briefly review some of the metabolic states in the human body. And there are two general states. You have the absorptive state in which the body takes energy and stores it and you have the post-absorptive state, in which the body takes this stored energy from the absorptive state and utilizes it. Now this absorptive state here is driven by the hormone insulin. Whereas the post-absorptive state is driven by the hormone glucagon. Now throughout the day, the human body will typically fluctuate back and forth between this absorptive state and this post-absorptive state. So to get a better understanding of how this looks, let's draw what I'll calla physiologic timeline. And let's just bring in a graph here to help describe this timeline. Now down here on this x-axis we'll have the time of the day. And right here in the middle we'll have noon, six in the morning, six at night, midnight, and then maybe we'll put three AM, nine AM, three PM, and nine PM. Now as I mentioned before, the body will fluctuate back and forth between this absorptive state and post-absorptive state. So let's see that here. And if you look closely, this fluxuation back and forth makes sense here, and around six AM when you go from this post-absorptive state while you're sleeping, and then you eat breakfast, and then you'll go into an absorptive state because you need to absorb the nutrients from the food in breakfast and then as your morning goes you go back into this post-absorptive state and so on and so forth. Now these changes back and forth between these metabolic states are driven by these hormones insulin and glucagon. So on the y-axis here, let's put in these hormone levels. So in purple here we'll put in insulin, and then in green we'll do glucagon. And what you can see from this is that it's really insulin here that's driving these changes between the post-absorptive state and the absorptive state. and glucagon also plays a role, but its level doesn't vary nearly as much as insulin's level throughout the day. Now since in type one diabetes the body doesn't produce enough of this insulin, it makes sense that the goal of treatment when we're treating type one diabetes, is to give insulin that will try and mimic the body's normal production of insulin. However, when we're treating type one diabetes, just giving insulin may be once or twice a day, as is done with most medication, doesn't really work because the levels are changing so frequently. 


So then how exactly do we manage type one diabetes? To get a better understanding of this, let's erase some of our work. Now fortunately, physicians and pharmacologists have created a very elegant method for treating type one diabetes. And this method is known as the Basal-Bolus Strategy. And in order to understand this concept a little bit better, let's first talk briefly about insulin. Now, insulin is a peptide hormone. And as such, that means when we give it as a medication, it can't be taken in a pill form, because the stomach and digestive system would break down the peptides or the protein of insulin into its component parts before it could be absorbed. And there for insulin must be given as an injection. And there are many different types of insulin that are available for use in the treatment of diabetes and they are classified based on how quickly they take effect, which is know as the onset of action and how long they work for, which is known as the duration of action. So to get a better understanding of this, let's create another graph similar to this one that we'll call the pharmacologic timeline. And on the x-axis here we'll put that duration of action. And this will be an hour, so we'll have maybe three, six, nine, 12, 15, 18 hours here. So one of the three main groups of insulins that can be given when treating type one diabetes are known as the rapid-acting insulins. And their pharmacologic time looks something like this. And these rapid-acting insulins usually take somewhere about 15 minutes to 30 minutes before the start working and their duration of action will last, you can see here, somewhere around four to six hours. 


Now the next major group of insulins are known as intermediate-acting insulins. And these intermediate-acting insulins, you can see by the graph, take a little bit longer before they have an onset of action, about 30 minutes to an hour, and then they last a little bit longer than the rapid-acting insulins, for somewhere between maybe eight to 12 hours, as you can see on the graph here. Now the last major category of insulin are known as the long-acting insulins. And as you can see on this graph, the long-acting insulins take even longer to take action, somewhere in the order of maybe one to four hours, and their peek action is not quite as intense as this rapid or intermediate-acting insulins, and their duration of action is much longer. Depending on the type of long-acting insulin, it can be somewhere between 12 and 24 hours. So now that we have a little bit better understanding of the different types of insulin and why it needs to be injected instead of taken as a pill, let's go back to this physiologic timeline here. And let's specifically look at this insulin level 
Now you notice that the insulin level never goes all the way down to zero. There's always this baseline level here. And we'll call this the basal level. And then intermediately there are these peaks, which we'll call boluses. And these boluses occur after we eat and they're what drive the transition from that post-absorptive state to the absorptive state, about three times a day, depending on how often you eat. Now hopefully what you can see by this is that if we transpose a couple of these graphs from the pharmacologic timeline onto the physiologic timeline, we can use injectable insulin to mimic this physiologic timeline in order to treat type one diabetes. So for these boluses, these kind of rapid peaks, you'll notice that they look somewhat like the rapid-acting insulin here. So let's put that on there. And then this basal level here, this constant level, you can create with a long-acting insulin. So we'll put that on the graph. Now hopefully what you can see by this, and it's starting to geta little crowded here so I'll highlight it, is that by using this Basal-Bolus Strategy someone with type one diabetes can kind of mimic the natural levels of insulin that the pancreas should be producing. And this is why this Basal-Bolus Strategy of treating type one diabetes is very efficient. Because it mimics what the body would do if the pancreas was working properly. So an overview of the Basal-Bolus Strategy is that usually once or twice a day, depending on the type of long-acting insulin, say in the morning and then again at night, someone with type one diabetes will take a dose of insulin, of this long acting insulin that will serve as this basal rate. And then at meal time they'll take an additional dose of the rapid-acting insulin to cover these boluses to help the body transition from the post-absorptive hereto the absorptive state, to absorb the energy in the meal they just ate. Now it's important to know that this graph demonstrates the principle of the Basal-Bolus Strategy, but it is somewhat of an oversimplification and that proper insulin management requires one to be very diligent with their insulin dosing and administration. 



This is especially important in regards to the bolus doses here. And this is because the amount of insulin that someone's gonna need to take with each bolus dose will vary depending on what their blood sugar is at that time as well as on how many carbohydrates they're planning on eating. So in order to properly manage their insulin regiment, individuals with type one diabetes must regularly check their blood sugar levels and adjust their insulin dosing accordingly. Now type one diabetes can be a very serious and potentially even lethal disease. 



However, with diligent adherence to the Basal-Bolus Strategy and regular appointments with one's physician in order to adjust the insulin dosing as well as monitor for complications, someone diagnosed with type one diabetes can still live a very healthy and long life.

Hi today we are going to discuss about one of the best way to control diabetes.
Yes it’s yoga but before we discuss about how it help to control diabetes first discuss what yoga is, how it developed, what was its origin.
So let’s start by
WHAT YOGA IS ??

Yoga is a mind and body practice with a 5,000-year history in ancient Indian philosophy. Various styles of yoga combine physical postures, breathing techniques, and meditation or relaxation.
In more recent years, it has become popular as a form of physical exercise based upon poses that promote improved control of the mind and body and enhance well-being.
There are several different types of yoga and many disciplines within the practice of yoga.
In the modern world, the South Asian art of yoga has expanded to all corners of the globe. While it is now a popular form of exercise and meditation, this has not always been the case.
Yoga has a long history of helping people achieve mental and physical balance.
There is no written record of the inventor of yoga.
Male yoga practitioners are known as yogis, and female yoga practitioners are called yoginis. Both practiced and taught yoga long before any written account of yoga came into existence.
Over the next five millennia, yogis passed the discipline down to their students, and many different schools of yoga developed as the practice expanded its global reach and popularity.
The “Yoga Sutra,” a 2,000-year-old treatise on yogic philosophy by the Indian sage Patanjali, is a guidebook on how to master the mind, control the emotions, and grow spiritually. The Yoga Sutra is the earliest written record of yoga and one of the oldest texts in existence and provides the framework for all modern yoga.
Yoga is well known for its postures and poses, but they were not a key part of original yoga traditions in India. Fitness was not a primary goal. Practitioners and followers of yogic tradition focused instead on other practices, such as expanding spiritual energy using breathing methods and mental focus.
The tradition began to gain popularity in the West at the end of the 19th century. An explosion of interest in postural yoga occurred in the 1920s and 1930s, first in India and later in the West.

Diabetes and Mindfulness
Mindfulness is the very essence of yoga, which basically means teaching yourself to be aware in the moment. Thoughts, movements, and emotions are all looked at from moment-to-moment, with no judgement or plan. It is also a coping mechanism and one that is very effective for diabetes management, as it helps people with diabetes come to terms with their illness and deal with it in a much calmer way. Mindfulness also helps people with diabetes adapt better to stress, depression and anxiety, and – in some cases – can help to eliminate these symptoms.

By avoiding negative thoughts and emotions and replacing them with feelings of contentment and even happiness, things like stress, anxiety, and depression are easier to handle. In terms of diabetes, it helps people to focus on the present and allows them to accept what they are feeling rather than try to change it. Mindfulness is effective in supporting diabetes management and the mental unrest that people with diabetes often face
How can Yoga Help Fight Diabetes?


Many studies have now shown that yoga is a great option for helping to fight diabetes. It can reduce high blood sugar levels and help blood glucose control. In addition, yoga helps to control stress, which also helps to decrease elevated blood sugar levels. Asanaspranayama (breathing exercises) and meditation is an easy and economical way to help fight diabetes. Not only does it aid in glycaemic control, but it also improves the overall quality of life. Furthermore, yoga helps reduce the risk of cardiovascular diseases, which may then reduce insulin resistance syndrome. Insulin resistance syndrome means that your body is not responding to the insulin that it naturally makes, which causes an increase in blood sugar level. This can then result in type 2 diabetes. The idea is to help regulate stress hormones, as this will help with both blood pressure and blood glucose levels.
Even after doing just one yoga session there will be a reduction in blood glucose levels
Pranayama
Pranayama is the practice of controlling the breath, which is the supply of our prana (life force). In Sanskrit, it translates to the extension of the prana (life force), or, breath control. Pranayama originated in India as a yogic discipline to help with concentration. There are various forms of pranayama, each of which works on cleansing, calming or relaxing the body and the mind in different ways.
Pranayama is highly beneficial in treating stress-related disorders as well as controlling glycaemic levels and overall quality of life.
Can Yoga Cure Diabetes?


Yoga works by rejuvenating the main glands that are involved with diabetes, like the stress glands and the pancreas. As mentioned above, certain yoga positions can rejuvenate pancreatic cells, as well as reduce stress. It can also help diabetes patients control their holistic manner by recognising the effects that things such as stress, a bad diet, and emotional imbalance can have on their well-being and their diabetes. That said, can yoga cure diabetes?
The yogic treatment restores the normal functioning of the pancreas and other glands of the endocrinal system. Almost every yoga pose has some sort of therapeutic effect on various organs and glands, including the pancreas. By choosing to do asanas that help revive the normal function of the endocrinal system, you can help restore the normal functioning of the pancreas. In the beginning, it is best to stick with asanas that are easily performed, and progress as your energy levels, flexibility and strength increase. We will get more into details on what yoga poses are best for diabetes later.
Yoga Asanas


The physical side of yoga will also improve the lives of people with diabetes as well as help fight it. Exercise (along with diet and medication) has been a foundation of diabetes management. Yoga asanas are often mentioned as a great form of exercise for diabetes patients, as it rejuvenates pancreatic cells, promotes weight loss, exercises the muscles and improves mental attitude. Basically, certain yoga postures can promote the production of insulin-producing beta cells, increase glucose uptake in muscular cells, improve weight control and help to create the right mental approach when it comes to dealing with diabetes.
Yoga asanas can help bring balance and health to all aspects of your life; physically, emotionally, mentally and spiritually. As such, it is an attractive alternative to traditional strength training regimes and aerobic exercises. Plus, it can be done right in your home with very little equipment. In terms of practicing yoga for diabetes, there have been numerous studies to prove that yoga has an enormous impact on glycaemic control, and as a result is beneficial in the management of diabetes. 

What Style of Yoga is Best for Diabetes?
We now know that a regular yoga practice will help reduce blood sugar levels, stimulate the pancreas, and lower blood pressure. Yoga is also proven to be excellent for weight loss. With obesity being a major contributing cause of diabetes, yoga for weight loss will definitely help in diabetes management. But with so many styles of yoga out there today, it is hard to know what style of yoga is best for diabetes.
Yoga for Type 1 Diabetes
Yoga is great for the body and the mind, and it also improves the working of endocrine glands and blood circulation – we also know that it helps to manage stress. Due to the fact that people with type 1 diabetes cannot naturally produce the insulin that is needed when they are stressed, blood sugar readings can get high, which is dangerous. Yoga helps to relax the sympathetic nervous system, which helps the endocrine glands. It also teaches you to embrace what you have and to let go of things that are out of your control.
Having type 1 diabetes is stressful enough on its own without additional everyday stress factors. So learning to accept the disease and not let it take over your life (i.e. depression, anxiety, and stress) is important. Yoga also promotes mindfulness, making you more aware of yourself inside and out, as well as your surroundings. In addition, it promotes healthy eating and mindful eating; meaning to control binge and/or emotional eating.
Type 1 diabetes is much harder to control than type 2, but regular exercise (like yoga) improves blood glucose regulation. This can then mean that daily insulin dosages can be reduced. It also decreases the risk of other complications than can occur in people with type 1 diabetes. Yoga will also improve vascular complications, as it reduces dyslipidaemia and helps to control oxidative stress.
People with type 1 diabetes need to be very careful not to do any strenuous exercise or more powerful forms of yoga. Slow-paced yoga is the best form of yoga for people living with type 1 diabetes.
The Best Yoga Poses for Diabetes
There are many styles of yoga and thousands of different yoga poses. When doing yoga for diabetes, it is important to do as many asanas as possible to stimulate the pancreas, as well as others that help you to lose weight, maintain a healthy weight and build muscle. Pranayama is an important part of yoga for diabetes and can be done on its own, or at the end of your yoga practice. These are the six best yoga poses for diabetes.
Ardha Vakrasana

Known as the Half Spinal Twist Pose, Ardha Vakrasana is a gentle twisting pose where the upper body twists, while the lower part of the body is being stretched. One of the main beneficial areas of this asana is on the waist and abdomen, as it stimulates all of the organs and glands in this part of the body, including the pancreas, the adrenal glands, and the sexual organs. It is also great for relieving constipation, backaches and spinal disorders.
Dhanurasana

When doing this pose, the pancreas gets fully energised, so it is a natural conditioner for it. Doing dhunurasana can potentially restore the pancreas back to health. This is particularly important for people with type 2 diabetes. As well, it is good for the thyroid, stomach ailments, and weight loss.
Uttanpadasana

Uttanpadasana is a pose that is done laying on your back and can be done one leg at a time or raising both at the same time. This asana corrects disorders of the pancreas, like diabetes, as well as working with all of the internal and external abdominal muscles. It can also have a great effect on constipation, indigestion and intestinal disorders. Uttanpadasana activates the entire nervous system and energises the inner cells.
Sarvangasana


Shoulder stand is an excellent pose for controlling and treating diabetes, as it massages the internal organs, which then activates the glandular-system. Sarvangasana also increases blood circulation so that your glands start to work to their full capacity. It is good for metabolism, increasing energy and regulating the thyroid glands. For people with diabetes, this pose will help control blood sugar, metabolise sugar and improve the function of the pancreas.
Kapalbhati 

Kapalbhati (fire breath) is a pranayama exercise that helps to cure and control diabetes when practiced regularly. It improves the strength and function of the pancreas and regenerates its beta cells. Another benefit of Kapalbhati is that it helps to lower sugar levels almost immediately.
Bhujangasana



Commonly known by many as cobra pose, Bhujangasana is extremely beneficial for people with diabetes. It tones the reproductive system, stimulates the internal organs and stretches the spine. It is also known to improve blood circulation, relieve constipation and energise the heart.

Food to avoid with diabetes.


Hi today we will be discussing about some of the food items that a diabetic should not eat.
I have usually seen that many people mistaking some of the food item thinking that they might not cause the blood sugar hikes but they do raise your sugar level.
In this article these food items are listed which have the potential to raise your sugar level.
So the list of such items are :-

White Bread


White sandwich bread is a refined grain, not a whole grain. When eaten as is, it has a high glycemic index and can directly lead to elevated blood-sugar levels. Swap white bread for whole grain or Ezekiel bread.
French Fries



French fries, for example, are a no-go. Fried foods are high in simple carbs and fat, which is a tough combination for diabetics. It will raise blood sugar quickly and keep it high for a long time because the fat takes a while to digest.
Sports Drinks


They can be an unnecessary source of calories, added sugars, and sodium, which are all things people with diabetes should avoid. Stay hydrated during your workout with cold H2O—it really is your best bet, or with a natural electrolyte-packed, low-calorie sports drink.
Soda
Within the first 10 minutes of drinking a soda, about 10 teaspoons of sugar hit the system. It's rapidly absorbed and signals an insulin release, which is a problem for people with diabetes since they have insulin resistance.
Diet Soda


Diet sodas aren't much better. Sweeteners are 200 to 600 times sweeter than sugar and signal an insulin release from the pancreas. So even if you're not drinking straight-up sugar, you're still signaling a release and perpetuating that dysfunctional physiological response.

Artificial Sweeteners
Contrary to popular belief, these fake sweeteners are not healthy for people with diabetes to consume. According to a study, drinking artificially-sweetened beverages daily was associated with a 36 percent greater risk for metabolic syndrome and a 67 percent increased risk for type 2 diabetes. Experts believe the reasoning is because artificial sweeteners are anywhere from 180-20,000 times sweeter than table sugar. Frequent consumption can cause an alteration in your sweet taste bud receptors, which makes vegetables and even fruits taste more bitter than they actually are. This causes you to neglect those foods and go after foods that satisfy that desire for sweetness.
Fast Food


It was found that those who ate fast food more than twice a week developed insulin resistance at twice the rate than those who didn't consume fast food. Insulin resistance increases the risk of developing type 2 diabetes and prediabetes.
Gluten-Free Foods
Gluten-free doesn't always mean the dish is healthier because the gluten protein provides elasticity and volume in baked goods, often times "gluten-free foods are actually denser and, therefore, will have more carbohydrates per serving [than conventional foods]."

Energy Drinks


It is found that consuming caffeinated energy drinks (with 200 milligrams of caffeine) can cause blood glucose and insulin levels to spike by upwards of 30 percent and may lead to subsequent problems bringing blood sugar levels down to normal. According to one of the researchers behind the study, the caffeine is at fault for your body's inability to subsequently stabilize blood sugar as the stimulant persists in your system for four to six hours after consumption. The underlying mechanism behind caffeine's influence on blood sugar is currently unknown.
Dried Fruit


Real fruit contains nutrients like water and fiber, which both help to fill you up. When dried, these sweet and chewy snacks can carry anywhere between 34 and 74 grams of carbs—for raisins and dates, respectively—for one small 1.5-ounce serving.
Orange Juice
It's natural and overflowing with vitamin C, but it's loaded with sugar—and totally void of any nutrients like fiber or protein to help slow the sugar spike. An average glass packs 36 grams of sugar—or about what you'd get from popping 4 Krispy Kreme glazed donuts into a blender and hitting frappe. What's more, most of the sweetness in juice comes from fructose, a type of sugar associated with the development of belly fat.
Muffins


It's a fat and calorie-mine, carrying over 520 calories and a third of the day's fat in one pastry. Foods rich in carbs, fat, and sugar are downright addicting. A study found that mice who had been fed diets with high levels of those very nutrients displayed withdrawal symptoms and were more sensitive to stressful situations after they were put on a healthier diet.
Cereal Bars
Granola- and cereal-based bars aren't just high in refined carbs, but they're also coated in countless grams of syrups and sugars to keep them bound. Cereal bars, in particular, can serve up nearly 30 grams of carbs and 16 of those are straight up sugar. With only 1 gram of fiber, your blood glucose levels will be hitching a ride on the sugar roller coaster.
Flavored Yogurts
Most fruit-flavored yogurts on the market contain little precious fruit and are sweetened with sugar instead. It's not just fruit-flavored yogurts that are the culprits. One well-known brand's Thick & Creamy Vanilla yogurt serves up 31 grams of carbs—28 of which are sugar and is only matched with 2.5 grams of fat and 7 grams of protein.
Potato Chips


There's a reason potato chips are one of the most addicting junk foods. They're pulverized, soaked in fat, and loaded with salt. Potato chips are a disaster for diabetes because they are a carb and fat bomb that will quickly spike your blood sugar.
Pancakes
Pancakes are a breakfast staple but they're almost completely void of nutrition. More of a pastry than a nutritious breakfast choice, pancakes are usually loaded with more unhealthy toppings such as sugary syrup and saturated fat-laden butter. Sugar and saturated fat in combination make the blood sugar spike last longer.
Fat-Free Frozen Yogurt
Vanilla frozen yogurt seems like the healthier alternative to ice cream, especially without all the fat, but when you remove that fat, you add more sugar for taste.This is an easy way to raise blood sugar. Not to mention getting fro-yo at a yogurt shop usually means you'll be adding tons of sugary toppings like candy, syrupy fruit, and hot fudge.

Skim Milk
Most people assume skim milk is better since all the fat has been taken out. But that's not the case for diabetics. When you take out the fat, this becomes a primarily carbohydrate beverage. A glass of milk at night will usually result in an elevated blood glucose level in the morning.
Vegetable Oil


Partially hydrogenated oils are a form of trans fat, which are some of the most inflammatory oils available. Watch out for vegetable oils that are made with partially-hydrogenated palm and soybean oils, which contain trace amounts of trans fats. Cook with extra virgin olive oil, avocado oil, or grapeseed oil instead.
Biscuits
Buttery biscuits aren't just pure carbs; they're also potentially laden with other bad-for-you things, too, especially commercial biscuits from the store or a fast-food joint. "If homemade, [biscuits are] still a flour and saturated fat bomb leading to inflammation of the body and brain," Anziani explains. In addition to diabetes, inflammation has been linked to other health complications such as obesity and cancer.
Sandwich Crackers
Although all-natural, unsweetened peanut butter is a good food for diabetics, it's a far cry from the processed peanut butter found in packaged sandwich crackers. These are made with highly refined crackers and peanut butter made with lots of sugar and salt which will raise blood sugar and blood pressure.
Mac & Cheese
Macaroni and cheese is usually made with white flour and highly refined noodles, mixed with high-fat milk and cheese. This combination of refined carbs and saturated fat will lead to inflammation in the body and brain. Macaroni and cheese also has a high glycemic load.
Flavored Instant Oatmeal
Plain oats are one of our best foods for diabetes because they contain a fiber called beta-glucan, which seems to have an anti-diabetic effect. However, flavored oatmeal is usually processed and contains added sugar and other sweetened ingredients. Pulverized oats are absorbed quickly, and adding extra sugar makes this into a sugar.
Instant Noodles
Instant noodles such as the packets of ramen and Cup-o-Noodles aren't just refined carbohydrates; they can contain up to a day's worth of sodium, too. This can spike blood sugar.
Grilled Cheese
Grilled cheese is often made with highly refined white bread and lots of fatty cheeses. Like mac and cheese, grilled cheese is nothing more than refined flour and saturated fat. This can lead to inflammation of the body and also spikes in blood sugar.
Jams and Jellies


Jam and jelly is a double-whammy for people with diabetes. It's just a sugar that is spread on another refined carbohydrate like bread or crackers and are usually devoid of any and all nutrition. [Using jelly and jam] is just a way of adding more sugar to something that already contains sugar.
Honey
Don't be fooled by the fact that honey is all-natural. It's still pure fructose, which is a sugar. It will spike blood sugar just like regular sugar will, and is still processed in the liver.
Popcorn


Though its base is corn, corn is highly glycemic and this snack is usually topped with artificial butter topping, caramel sauce, or more unhealthy items. And yes, even stovetop popcorn is still a carbohydrate-ridden snack.
Packaged Baked Goods
Packaged baked good contain tons of added sugar, usually in more than one form: sugar, high fructose corn syrup, and dextrose, to name a few. They're also packed with artificial ingredients and made with partially hydrogenated oils, which are inflammatory trans fats. These processed sweets will spike blood sugar and lead to inflammation.
Ice Cream


Aside from all the sugar, ice cream is usually packed with artery-clogging saturated fat. It can also be a problem for people who mindlessly eat. Ice cream activates pleasure centers of the brain, blows out dopamine receptors, [and leaves you] wanting more and more. Many will eat the whole container.
